FAMILY SUPPORT TEAM

 In Home Support Services

 Program Philosophy

In Home Support Services provides intensive in-home services to assist families in maintaining a safe and stable home, in which their children can remain without fear of being removed. Services provided will be directly linked to the family’s safety plan as it is designed by DCF/FFN. We will provide weekly progress notes to the unit that referred the family (Protective Services or Protective Investigations).

Wraparound Services 

Program Philosophy

Wraparound Services provides less intensive in-home services for families utilizing the Wraparound model. This model focuses on the family’s strengths and needs as they are prioritized by the family. The team then works with the family to map out ways to meet those needs, possible barriers to meeting their needs, ways to overcome those barriers, and increasing the family’s informal support system. Wraparound Services can include support services, family team conferencing, and resource development, etc. We will provide weekly progress notes to the unit that referred the family (Protective Investigations).

 Population Served

Families in Walton County that have involvement with Department of Children and Family Services and Family First Network and have had children placed in their care are eligible to be served in the FST Relative/Non-Relative Program.
