Appointments

If you would like to schedule an appointment, please call our friendly outpatient staff at (850) 892-8045.  Our front desk is open Monday through Friday 8:00 am - 5:00 pm. Appointment times are available during the hours of 9:00 am - 4:00 pm.  If you find it necessary to cancel or change your appointment, please do so at least 24 hours in advance.  Your assistance in keeping your appointment or notifying us is greatly appreciated! 

Transportation Services: Free transportation to any non-emergency healthcare-related appt is available under both Humana and Molina. Members are encouraged to schedule at least 24 hours in advance.  HUMANA: (Modivcare) 866-779-0565  or MOLINA (Modivcare) 866-528-0454
